File of Life
File of Life is a life-saving program that supplies citizens with a small form that goes into a cardholder to be hung on your refrigerator door provides emergency personnel critical contact and medical information in case of an emergency in the home. Click here to download the File for Life form. Forms and cardholders can also be acquired by contacting the EMS department, or filling out the form below. We strongly encourage everyone, especially elderly citizens and those with chronic medical conditions to keep a File of Life on their refrigerator door.
