Leave of Absence:

It is possible that at one time or another, you may need to request a Leave of Absence under the Family Medical Leave Act or the Vermont Parental Leave Act. Should this ever be the case, there are several forms you will need to complete and return to the Human Resources Department. You can learn more about Leaves of Absence by reading these two documents: FMLA Employee Rights and the FMLA/VPFL Guidelines. Once you've had a chance to review these documents, you can then download and complete the following form:
